How to Order

Our programs are available exclusively through the Health Sciences Consortium. Mail prepayment, or FAX or mail a purchase order with the following information:

• title of each program
• catalog number of each program, e.g. A900-CB-010
• price (or preview charge) of each program
• number of copies of each program
• shipping and handling charges (will be billed at time of shipment if not included)
• shipping address
• billing address if different from shipping address
• name and phone/fax numbers of a contact person.

HSC does not accept phone orders and is not set up to charge orders on credit cards. Call HSC’s Customer Service Department to confirm availability of alternative formats before placing your order.
